Lightbeam-powered REACH ACOs deliver higher savings, lower readmission rates, and improved care outcomes for Medicare beneficiaries
Lightbeam Health Solutions, the leader in population health enablement technology and solutions, proudly announces that Lightbeam-powered Accountable Care Organizations (ACOs) engaged in the CMMI (Centers for Medicare & Medicaid Innovation Center) Realizing Equity, Access, and Community Health (REACH) program achieved exceptional results in Performance Year 2023 (PY23), generating over $110 million in gross savings.
In PY23, Lightbeam-powered REACH ACOs drove significant cost savings and improvements in clinical performance, demonstrated by stellar outcomes when compared to REACH ACOs not leveraging Lightbeam:
- $22 million in average generated gross savings
- 45% higher average gross savings
- 24% higher average earnings
- Lower readmission rates and all-cause admission rates.
The ACO REACH model by the CMMI aims to advance health equity, improve access to high-quality care, and reduce healthcare costs for underserved populations. ACO REACH organizations bring a targeted approach to value-based care (VBC) as a crucial part of the broader Medicare Shared Savings Program (MSSP) framework, enabling providers to balance cost savings with improved patient outcomes in communities with limited access to care. REACH ACOs differ from MSSP ACOs by managing smaller patient populations, taking on higher financial risk, and focusing on underserved communities, all within a model that emphasizes advanced care coordination, health equity, and accountability for cost and quality outcomes. Lightbeam empowers REACH ACOs with an advanced solution suite to identify specific health equity vulnerabilities within communities and automates patient monitoring at scale to ensure the most at-risk patients receive the resources and care they need at the right time.
“Lightbeam has supported ACOs in navigating this transformative model since program initiation. By equipping them with tailored strategies to address social vulnerabilities and balance quality and claims performance, we prioritize impactful, equitable care delivery for Medicare beneficiaries,” said Pat Cline, CEO at Lightbeam. “The results achieved in PY23—surpassing benchmarks and delivering significant savings—highlight both the dedication of ACO REACH participants and Lightbeam’s continued partnership in advancing health equity while improving quality and cost.”
Looking ahead to PY24, Lightbeam remains focused on supporting ACO REACH organizations in successfully addressing health disparities to transform the healthcare landscape. As healthcare organizations continue to navigate the VBC transition, Lightbeam serves as a trusted partner in providing innovative solutions that drive measurable success. Lightbeam has led the way in driving value for client-partners, helping to generate more than $3 billion in gross MSSP savings since program inception and more than $5 billion in total savings across all value-based care contracts.
